Description
N-terminal pro-brain natriuretic peptide (NT-proBNP) is a marker of heart failure. When cardiomyocytes are stimulated by pressure or stretch—such as when ventricular volume expands and pressure load increases—a 134-amino acid precursor B-type natriuretic peptide (pre-proBNP) is first formed. Protease then cleaves the N-terminal 26-amino acid signal peptide, converting it into pro-B-type natriuretic peptide (proBNP) containing 108 amino acids. This proBNP is further cleaved by endonuclease into an N-terminal NT-proBNP containing 76 amino acids and a cyclic structure containing a 32-amino acid BNP. Both polypeptides are released into the bloodstream.
The biological half-life of NT-proBNP is 60-120 minutes, whereas the biological half-life of BNP is 18 minutes. Detecting NT-proBNP is more stable and accurate.
