Description
Troponin (Tn) is a regulatory protein for muscle contraction, located in the fine filaments of contractile proteins, where it regulates muscle contraction and relaxation. It has three subtypes: fast-response, slow-response, and cardiac troponin (cTn). The first two are linked to skeletal muscle, while cTn is exclusive to cardiomyocytes and consists of three subunits: troponin T (cTnT), troponin I (cTnI), and troponin C (cTnC). cTnI, unique to cardiomyocytes, contains 210 amino acids and has a molecular weight of 23.88 KDa. When cardiomyocytes are damaged, cTnI is released from myocardial fibers. Elevated cTnI levels in serum indicate cardiomyocyte injury, with higher specificity and sensitivity than standard myocardial enzyme profiles. Troponin is a key marker for myocardial injury and necrosis, playing a critical role in diagnosing and risk-stratifying acute myocardial infarction (100% sensitivity, 91% specificity, and long detectability).
